Your Imperial Hospice and Palliative Care team directly works with your physician and our hospice medical director to ensure person-centered care. Our palliative care approach allows us to render the physical, emotional, and spiritual support you and your loved ones need during this journey.
Our Care Team and What They Do
Supportive Therapies
- Provide nutritional counseling, wound care, and other therapies as directed by the interdisciplinary team.
Attending Physician
- A licensed doctor designated by the patient as the physician responsible for his/her medical care.
- Participating member of the interdisciplinary team.
Medical Director
- Co-certifies terminal prognosis with the attending physician.
- Participating member of the interdisciplinary team.
- Participates in the development of the individual plan care.
- Consults with other physicians regarding pain and symptom management, as requested.
Skilled Nursing
- Identifies the patient’s/family’s physical, psychosocial, and environmental needs.
- Addresses symptom management and comfort care needs.
- Supports and instructs family.
- Documents problems, appropriate goals, interventions, and patient/family response to the plan of care.
- Coordinates patient/family services.
Certified Home Health Aide
- Assists patients with hygiene, feeding, light housekeeping, and similar personal care activities as determined by the individual plan of care.
Volunteers
- Assist the healthcare team in providing optimum quality life to terminally ill patients and their families.
Spiritual Support Staff
- Provides spiritual care services that are consistent with patient/family beliefs and desires.
- Communication and support.
- Consultation and education for patient/family and interdisciplinary team members, including grief and bereavement services.
Social Worker
- Provides counseling and linkage to community services for patients/families to assist them in developing optimal coping strategies.
In addition to the services of your hospice interdisciplinary team, Imperial Hospice and Palliative Care also provides:
